Medical Record Release Form. This form must be filled out completely before Grayson and Associates can send your records to another party. We only accept our medical record release form.
Medical Record Request Form. This form can be used to request that your records from another practice be send to Grayson and Associates. This form should be sent to the party from who you are requesting the records. Please do not send this form to Grayson and Associates.
